Make a document part of the form
Add a PDF Form widget, select the source PDF and place answer fields over the document. Configure each overlay's key, position, size and validation. Check each page at the intended display scale.
Use a stable source file URL. The source PDF is the blank template; the completed submission document is a different file and should follow the site's private attachment rules.
Try the working example
- Open the PDF example.
- Fill the displayed document using fictional values.
- Use the configured export/submit action.
- In the Umbraco backoffice, open that form's Submissions and inspect its PDF attachment.
The blank example PDF is public. Completed submission PDFs are retrieved through the authenticated file endpoint. Their identifiers are not public download permissions.
Understand the output
This example places an HTML input layer over a PDF and produces a completed flattened copy. It does not require the blank file to contain AcroForm fields, and the completed output is not promised to retain editable PDF widgets.
Verify the exported document visually: text positions, multiline values, page count and clipping. Also check that the submission table displays a useful file link rather than a JSON object.
See Uploads & storage for limits and authorised downloads, and Submissions for entry inspection.
